The Cleveland Board of Control consists of members of the mayor’s cabinet and leaders of city departments. City Council passes some legislation – particularly legislation that authorizes the city to enter contracts for various purposes – that leaves vendor selection and compensation to be set by the Board of Control. 2020 health care costs were ~$100 million. With prescription costs ~$110 million.
These contracts are well in excess of those actuals, so it sounds like they're overbudgeting to provide a cushion if needed.
